It is in your best interest to know and understand your responsibility for any deductibles, co insurance and other out-of-pocket expenses prior to your appointment. Our Practice abides by the contractual and legal obligations if health benefit plans to collect charges resulting from out-of-pocket expenses. We do recognize that circumstances may arise where an individual is unable to pay their balance in full, we have established a payment plan that allows you to pay the balance over a predetermined period of time.
